What is the background of the project?
Prediabetes is a hyperglycemic state between normal glucose tolerance and diabetes. The Expert Committee on Diagnosis and Classiﬁcation of Diabetes Mellitus identiﬁed an intermediate group of people with glucose levels that do not meet criteria for diabetes, yet having higher than normal. These individuals were deﬁned as having impaired fasting glucose (serum fasting glucose levels between 100mg/dl and 125 mg/dl) or impaired glucose tolerance two hours post load glucose level during oral glucose tolerance test between 140 mg/dl and 199 mg/dl. Insulin resistance defines as a decreased sensitivity and responsiveness to the metabolic actions of insulin. Insulin resistance predisposes to several disorders such as hyperglycemia (diabetes mellitus), hypertension, and dyslipidemia, In addition, insulin resistance contributes to vasoconstriction, inflammation, and thrombosis, leading to accelerated atherosclerosis.
What is the aim of the project?
1. Evaluate the prevalence of insulin resistance among individuals with prediabetes 2. Ascertain the association between Triglyceride glucose ratio and Triglyceride / high density lipoprotein-cholesterol ratio as marker for insulin resistance among individual with prediabetes
What techniques and methods are used?
A case control study which its protocol involves 150 individuals, 100 are diagnosed with newly prediabetes and another 50 apparently healthy volunteers (control) who will be sex and age matched and will be select from medical staff in emergency teaching hospital. The individuals will be recruited from Azadi General Teaching Hospital, Department of endocrinology in Duhok city. A pre-test questionnaire will be done to get information on: name, age or date of birth, gender, and blood pressure, family history of prediabetes and diabetes mellitus, family history of dyslipidemia, and family history of cardiovascular disease. Anthropometric measurement will take as height, weight, body mass index, and waist circumferences. An overnight fasting blood samples will use for biochemical analysis: the diagnosis of prediabetes will depend upon oral glucose tolerance test, with measurement of the serum lipid profile, glucose, insulin, and glycated hemoglobin test (HbA1c) for all subjects. Data entry will be put into the SPSS system, results of statistical analysis using statistical methods like descriptive statistics will be done in the form of (frequencies, cross tabs) and correlation methods from SPSS and Excel will be performed with interpretation and conclusion of results in the form of tables and graphs, and comparison with other regional and international studies will be made.
What is the role of the student?
- The student will observe the practical experiments but will be highly involved in the analysis of the results
- The tasks will be done under supervision
What are the tasks expected to be accomplished by the student?
The students will be taking history and based on the information collected a survey will be filled which includes results of history taking and examination, as well as results of lab tests done including measurement of the serum lipid profile, glucose, insulin, and glycated hemoglobin test (HbA1c). Therefor he will basically do rounds with on patients and he will also be taking blood samples from the from them for the tests mentioned above. Furthermore, he will be asked to make data entry into SPSS and observe the analysis process while do some literature review in the subject matter to enhance their knowledge and critical reading of research papers. In the paper writing, the student will mainly observe and helped if he had any questions and concerns regarding academic writing.
Will there be any theoretical teaching provided (preliminary readings, lectures, courses, seminars etc)
No there will be no theoretical teaching
What is expected from the student at the end of the research exchange? What will be the general outcome of the student?
- The student will prepare a scientific report - The student’s name will be mentioned in a future publication
What skills are required of the student? Is there any special knowledge or a certain level of studies needed?
working with supervisor in lab for enhancing preparation of sample as well as gaining communication, history taking and physical examination skills.
