What is the background of the project?
Chronic wounds become a social, economic, and public health problem and need advanced therapy. Mesenchymal stem cells (MSCs) represents a promising alternative therapy for chronic ulcers. The impacts of MSCs are chiefly identified with paracrine properties, opening opportunities for exosomes-based treatments. Platelet-rich plasma (PRP) has been used in treatment of chronic wounds as it contains plenty of growth factors secreted by platelets. The exosomes derived from PRP (PRP-Exos) have been demonstrated to epitomize essential growth factors from platelets.
What is the aim of the project?
The aim of this study is to assess the regenerative and healing capacity of autologous adipose-derived mesenchymal stem cells (MSCs) together with their exosomes combined with autologous PRP with its exosomes in chronic unhealed ulcers.
What techniques and methods are used?
100 patients with chronic ulcers are divided into five groups randomly. The first group: in which 20 patients will receive the conventional treatment for chronic ulcers. The second group: in which 20 patients will be injected with autologous adipose derived MSCs with their exosomes in their ulcer edge and base. The third group: in which 20 patients will be injected with autologous PRP with its exosomes in their ulcer edge and base. The fourth group: in which 20 patients will be injected with combined therapy MSCs and PRP combined with their exosomes in their ulcer edge and base. The fifth group: 20 patients will be given MSCs injection with their exosomes while the ulcers will be covered with PRP membrane, with adipose derived MSCs cultivated on it. In every group, the injections will be repeated every week for four injections. The MSCs prepared by aspiration of adipose tissue by abdominal liposuction from the patients then undergo tissue cultures to expand MSCs. Exosomes were obtained from supernatants of MSCs, after ultracentrifugation, the Purified MVs cultured overnight in the medium used for collection of MVs. The PRP was prepared by mean of a two-step centrifugation preparation of a blood sample, which was treated with an anticoagulant. By this process the platelet concentrated approximately fourfold to sevenfold as compared to untreated whole blood. PRP exosomes were prepared by ultracentcentrifugation of PRP several times then the supernatant used. The patient’s assessment is done every week for one month to assess size of the wound, time for closure, vascularity and the state of granulation tissue formed and the healthy skin around the ulcer. All data will be prospectively collected before and during treatment and follow-up.

1- Student will be able to examine the different types of chronic ulcers and notice the difference before and after transplantation. 2- The student will be able to separate stem cell and identify the characterization of it. 3- The student will be able to deal with tissue culture. 4- Students will observe patient’s assessment which is done every week for one month to assess size of the wound, time for closure, vascularity and the state of granulation tissue formed and the healthy skin around the ulcer.

The project include many workshops about regenerative medicine and molecular biology.
