ADDITIONAL DOCUMENTS REQUIRED BY SPECIFIC LOCAL COMMITTEES:
ANCONA: Hepatitis B vaccine (if there is no proof of it in the Vaccination Card), Tuberculosis test (Mantoux), Antibody titer for Hepatits B (HBV), measles, mumps, rubella, chicken pox, pertussis. If serologic testing indicates that you are not immune from the aforementioned diseases, then you have to get MMR (measles, mumps, rubella) vaccine (even if you are not immune for one or two of these three diseases), and chicken pox vaccine, unless you have a proof of previous vaccination against these diseases in the Vaccination Card.
BARI: pregnancy test, tbc test
BRESCIA: A clean bill of health signed by your family doctor and written in ENGLISH. Health insurance, liabily insurance, accident insurance.
CAMPOBASSO: health insurance, liability insurance, Origin university's photographic identification card requested.
CATANZARO: Tuberculosis test (Mantoux), Hepatitis B vaccine (if there is no proof of it in the Vaccination Card).
FERRARA: The LC requires a students' policy to be drawn up directly at their International Students' Mobility Office, therefore contact the LEO/CP before taking any decision about insurances.
GENOVA: Certificate of vaccination against Tuberculosis (if there is no proof of it in the vaccination card but you have been vaccinated against it). If you haven't been vaccinated against Tuberculosis, you need to get a Mantoux test. If the test is positive, you have to get a chest X-ray (which certifies the absence of active infection), then you have to upload the result on the Card of Documents. To attend any pediatrics department, it's mandatory to provide: Hep A Antibodies, HepC Antibodies, HIV Antibodies (npt mandatory), Chickenpox Antibodies, Measles + Rubeola + Whooping Cough Antibodies. It's also required that your GP/Competent Doctor fills an international vaccination card provided by the LEO within your CA.
MILANO: HBsAg count, HBV vaccination, Tuberculosis test (Mantoux) done at the earliest 12 months before the exchange (no sooner). If the test is positive, you have to get a chest X-ray (which certifies the absence of active infection), then you have to upload the result on the Card of Documents.
MODENA: probably a visit to arrival will be required.
MONZA: There is the possibility to draw up the Liability Insurance with the University insurance company. For more informations ask the LEO/CP. You will have to draw up the remaining policies on your own. Before their arrival, students must provide a letter from their family or occupational physician that must be typed on the doctor's letterhead and in legible English. This letter should inculde: - Immunity (to be shown by laboratory tests) and certificate of vaccination towards the following disease: hepatitis B, measles, mumps, rubella and varicella; - Date of the most recent (within the last two months) tuberculin skin test (PPD) and the end result of reading (positive or negative; if positive the diamer of induration in mullimeters). If tubercolin skin test is positive, a recent (within the last two months) chest x-ray should be provided.
NOVARA:You have to get Polio vaccine, TD (tetanus, diphtheria) vaccine, Hepatitis B vaccine if there isn't any proof of them in the Vaccination Card. If you got Tuberculosis vaccine recently or in the past, you have to upload a valid certification about it in the Card of Documents.
PADOVA: the insurance for the students of the University of Padua will be paid by the LC and upon arrival the student will have to repay the LC. The insurance amounts to 8.50 euros.
PALERMO: Some departments are not available according to the LC page explore.
PARMA: vaccination required: anti-poliomyelitis, anti-diphtheria, anti-tetanus, anti-hepatitis B, anti-meningococcus B, anti-meningococcus C, anti-varicella, anti-morbid, anti-pertussis, anti-rubella, anti-haemiphilus influeae B, anti -mumps.
PISA: Vaccination required: HBV, morbillo, parotite (mumps), rubella, varicella, tetanus, meningococcus C.
ROMA UCSC: SMA 12 (comprehensive metabolic panel / chemical screen), blood count, urinalysis, markers of exanthematous diseases (rubella, measles, chicken pox, mumps), markers Hepatitis B and C (HBV, HCV), Tuberculosis test, done maximum 3 months before the arrival (Mantoux). If the Mantoux test is positive, you have to get a Quantiferon test and a chest X-ray (which certifies the absence of active infection), then you have to upload the result on the Card of Documents. ALL THE DOCUMENTS MUST BE IN ENGLISH (you can send your documents to a certified translation service if your health facility doesn't provide you with an english language version of the results of your medical exams).
SALERNO: vaccinations required for Hepatitis B, poliomielitis, DPT (difteris, pertussis, tetanus).
TORINO: HBsAg count. You have to get a Mantoux test or a BCG vaccine or IGRA or RX if there is no proof of them in the Vaccination Card or in an official certificate, medical certificate of passing chickenpox.

You just need to put an “x” in the “Modulo di Richiesta Tirocinio” where it asks about attachment n.2. 4. Scheda Attività Rischi (attachment n.3): you will complete this document with your personal details . The second part of this document will be fullfilled from your chief department. Just remember to put an “x” on “attachment n.3” that you will find in the “Modulo Richiesta Tirocinio”. 5. Informativa Documentazione Sanitaria (attachment n.4): this paper will be attached to the “Modulo Richiesta Tirocini”, and it sais you will send us: · Vaccination Record · Mantoux Test for tubercolosis · Medical certificate for markers HBsAg, anti HBs, anti HCV (done in the last 12 months) · Medical paper for HIV antibodies (done in the last 12 months) You MUST send these medical exams together with all the other papers you have to sign. 6. Insurance: the insurance MUST include -Accident Insurance: a policy against the risk of personal accidents which may occur during your clerkship. This part must cover “death and permanent invalidity” too. -Liability Insurance (third-part liability): it is about civil responsibility in case you cause damages to others. The LC specify the need for appropriate clothing in the hospital or laboratory (closed shoes, long pants / skirts, covered shoulders); the incoming must bring with him the stethoscope and the gown (as per national EC) - It will be strongly recommended a level of Italian A2 or higher for SCOPE - In terms of preventive medicine will be required to send the vaccination booklet (as per national EC ) with subsequent eventual request for integration according to university requests; the incoming will also have to execute a Mantoux or Quantiferon Gold not earlier than 2 months after its arrival, and if positive a chest plate is required to rule out active disease - Curriculum Vitae, proof of enrollment and student record (as per national EC) - For each department available you will need to have passed the corresponding exam - Health insurance, accident insurance and liability insurance.L'AQUILA:
